Support to stay in the family home

Coordinated Family Support is designed for adults with developmental disabilities who are 18 or older and choose to keep living in their family’s home, rather than move to a residential facility or live independently.

Rather than replacing a family’s existing service coordinator, CFS works alongside them helping coordinate the different services and supports a family already relies on, and helping think ahead to long-term needs, so the person can keep living well at home as circumstances change over time.

Program Goals

Home is where many people feel most themselves.
We help families make it work, today and in the years ahead.

Bringing together the different supports and services a family already uses.

Helping families think ahead and plan for long-term needs.

Linking families to helpful programs and community resources.

Helping the person continue living well in their family’s home as needs change over time.

Training and guidance so caregivers can meet their loved one’s daily needs with confidence.

Helping the person and their family take part in social, recreational, and leisure activities.
