Support to stay in the family home
Coordinated Family Support is designed for adults with developmental disabilities who are 18 or older and choose to keep living in their family’s home, rather than move to a residential facility or live independently.
Rather than replacing a family’s existing service coordinator, CFS works alongside them helping coordinate the different services and supports a family already relies on, and helping think ahead to long-term needs, so the person can keep living well at home as circumstances change over time.
